Medication Reconciliation
Siemens delivers Medication Reconciliation as a stream of enhancements to Advanced Patient Assessments/Clinical Observations and Results, and the Lifetime Clinical Record® (LCR) Med List. It provides model pathways that enable care teams to: 

• Collect home medications
• Reconcile medications at admission, transfer, and discharge
• Write inpatient orders using the home med list
• Print discharge prescriptions

Supporting reports— Medication Reconciliation also includes reports to support the medication reconciliation process at time of admission, transfer, and discharge. 

Computerized Physician Order Entry (CPOE)— Physicians working with CPOE can write inpatient orders using the home med list to improve their productivity and efficiency.

INVISION^® MedsConnect works with Medication Reconciliation.
INVISION MedsConnect is a content subscription service that provides clinicians with access to 12 months of a patient’s prescription history, which can be used in an inpatient, outpatient, or emergency room setting.
